From Ygnacio Valley Road:

From I-680, drive east approximately 3 miles to Lennon Lane.  Turn left on Lennon Lane and make another left into the driveway.  Dr. Bortz's office is in suite 255 on the second floor.

 If you take the stairs up, you can walk straight to the entrance of the office.  

If you take the elevators, the entrance to the office will be on your left.
